- By mail – Mail the completed form and a check for $6.50 to the NCDMV Traffic Records Branch, Crash Reports Unit, at 3106 Mail Service Center, Raleigh, NC 27697-3106. Allow approximately 10 business days for the NCDMV to process your request.
